Chronic Disease Management is the coordination and care of treatment for patients with chronic or terminal medical conditions such as coronary heart disease, stroke, lung cancer, depression, Type 2 diabetes, asthma or osteoporosis. While many chronic diseases cannot be cured, they can be managed, so it's important to build a long-term care plan with a healthcare professional.

COVID-19 UPDATE: All persons attending a medical appointment at a Sonic HealthPlus clinic will be required to complete a COVID-19 Triage Assessment form. If you are not vaccinated or fully vaccinated, we will need to undertake a risk assessment, which may include Rapid Antigen Testing.
